Chronic Disease Nurse
Valentine Family Medical Practice is currently seeking a patient-focused Chronic Disease Nurse to join our nursing team
Responsibilities include, but are not limited to
Management of Chronic Disease patients in collaboration with treating GP
Preparation of Care Plan and Team Care Arrangement documentation
Consulting with the patients prior to seeing the GP with a strong focus on optimising lifestyle management
Position Description: We are currently seeking a patient-focused Chronic Disease Nurse to join our nursing team
Responsibilities for Chronic Disease Management include, but are not limited to
Management of Chronic Disease patients in collaboration with treating GP
Preparation of Care Plan and Team Care Arrangement documentation
Co-ordination of annual Diabetes Cycle of Care for allocated patients
Consulting with the patients prior to seeing the GP with a strong focus on optimising lifestyle management to improve overall health, optimise management of current comorbidities and reduce the likelihood of developing further comorbidities
Hours per week: Wednesday and Friday, 9am -6pm Chronic Disease Management plus half a day prep time
Option of additional hrs/days including working in the Treatment Room and Chronic Disease Management
Availability for holiday and sick leave relief
Responsibilities in Treatment Room include, but are not limited to
Vaccinations and injections, including childhood and travel
Assisting in minor procedures
Sterilization of equipment
Health Assessments
RMS Medical checks
Wound care
ECGs
Ear Syringing
Management of vaccines, stores
Infection control
Essentials Description:
Current AHPRA Registration as a Registered Nurse
Current Working with Children Check (or ability to obtain)
Experience in General Practice nursing in a Chronic Disease Nursing role and Treatment Room role
Friendly, patient, responsible, open to learning, attention to detail
Able to give quality, evidence based patient care
Computer skills
Communication skills
Desirables Description:
Current CPR certification, or willingness to obtain
Post-graduate study in Chronic Disease Management and if required a willingness to attend external training
Experience with Best Practice software
Current ear syringing certification, or willingness to obtain
Accredited Immuniser
Venepuncture skills

Valentine Family Medical Practice is a long established medical practice in the village-like suburb of Valentine on Lake Macquarie
We currently have 5 Clinical Consultation Rooms and a Treatment Room.
Staffing consists of 4 Nursing staff and 5 Medical staff , 2 Administration and 5 Reception staff
Staff facilities include an expansive first floor staff lunchroom overlooking Lake Macquarie and a nurses’ preparation office for non-patient contact work.
