Funding announced by the Federal Government under the Building a Stronger Medicare
package will support PHNs to commission multidisciplinary teams to improve the management of chronic diseases in the primary care setting.
Multidisciplinary teams involve multiple health professionals from several different disciplines who care, collaborate and communicate together to provide comprehensive care for the patient/client. See Multidisciplinary team - Integrated care (nsw.gov.au)
The Multidisciplinary Team (MDT) Pilot Program aims to support GPs in the management of chronic conditions, improve health outcomes and reduce avoidable hospitalisations by:
- Increasing access to the right services at the right time;
- Reducing barriers to accessing care; and
- Increasing health literacy and self-management.
Health professionals that can be commissioned to form part of the multidisciplinary teams include:
- Allied Health Professionals
- Aboriginal Health Workers and Practitioners
- Nurses, including Nurse Practitioners and Midwives
- Assistant Health Workers (such as Allied Health Assistants)
The PHN will commission two MDT pilots in the identified priority locations of Moree
and Muswellbrook to address locally identified chronic health needs.
Providers who are based in or have the capacity and capability to establish and deliver face to face multidisciplinary team-based clinical services in one or both priority locations are invited to respond to the tender.
