Quality Improvement Resources
The Primary Health Network has produced a number of clinically specific quality improvement (QI) resources that are available to you to support your work in QI in your practice.
The Primary Health Network can provide you with QI toolkits and one-pagers, along with examples of implemented QI activities in a number of clinical areas including diabetes, Aboriginal health, mental health, suicide prevention, health assessments, CVD, smoking cessation, height and weight, screening activities and immunisation.
Please liaise with your Primary Care Improvement Officer to ask for more information and for access to these resources, and/or join the PHN’s ‘Primary Care Quality Improvement Community of Practice’ where the resources are also available.
