Health Care Homes

A ‘home base’ to coordinate care for patients with chronic and complex conditions.

Our PHN participated as part of the Commonwealth Government’s Health Care Homes pilot. This initiative is aimed at providing better integrated care to Australians with chronic and complex conditions.

RACGP research reports that 772,000 preventable hospital admissions each year could be reduced by improving the quality and role of primary care (RACGP Sustainable Primary Healthcare System Report 2015).

Health Care Homes are a ‘home base’ that coordinate the comprehensive care that patients with chronic and complex conditions need on an ongoing basis.

Key benefits of a Health Care Home include:
  • Enabling a model of care that works across the whole health system and integrates GPs and other primary health care clinicians with hospitals and community services in a manner that is seamless for patients
  • Promotes a partnership model between health care professionals that tailors care to the specific needs of patients
