Shaping the future of healthcare in Glen Innes

Glen Innes Health Hub

The Hunter New England and Central Coast Primary Health Network (the PHN) is working with the Glen Innes community to implement an innovative pilot healthcare model, known as the Glen Innes Health Hub, as part of the Better Health for the Bush Framework.

The Health Hub aims to meet community needs through a team-based approach and may introduce new or enhanced services. It will be supported by digital solutions and assistance to navigate the health system effectively.

The Health Hub services may encompass allied health, social services, aged care, disability, and primary care services, including GP care.

The Glen Innes community has provided valuable feedback and insights through a Community Town Hall Information session held in March, a series of community consultation sessions in May and the recent Have Your Say – Glen Innes Health Hub Survey.

Findings from the survey will assist with the planning of the services and supports to be delivered through the health hub, and next steps will be shared later this month.
