General Practitioners, Allied Health, Nursing and other primary care professionals are invited to join us at Belmont 16s Sailing Club for the PHN’s inaugural Palliative and End-of-Life Care Symposium being held on Thursday 13th & Friday 14th June, 2024.

The Symposium consists of two key events:

  1. General Practitioner Dinner, Thursday 13th June, 6:00pm-8:30pm.
    The dinner will include a presentation by our keynote speaker Professor Liz Reymond on home based palliative care.
  2. Education Day for all health care professionals, Friday 14th June, 8:30am – 5:00pm. This will include a day of presentations, case studies and panel discussions as well as a range of stall holders. This day will be livestreamed for those unable to attend face-to-face.

The Palliative & End-of-Life Care Symposium themes include:

  • Increasing choice and autonomy for more people to be cared for and die at home, if that is their preference
  • Improving equity of access to the best palliative care services at home
  • Supporting people to manage their end-of-life journey in the community and reduce unnecessary hospital visits
  • Exploring innovative models of palliative care
  • Providing best practice person-centred palliative care for diverse populations across the Hunter, New England & Central Coast region.

Jane Gray

Health Service Leader, Facilitator and Meditation Teacher

Jane is an experienced health service leader, facilitator and meditation teacher. As MC for the Education Day on June 14, she brings deep experience in exploring and improving the experience of palliative and end of life care for patients, families and health service professionals. Jane has 30 years’ experience in senior executive leadership, strategy, facilitation and communications. Jane is a Non-Executive Director on Boards in the health, education and charitable sectors. She also provides training and facilitation to private, government and not for profit clients. She is a practitioner and teacher of mindfulness and meditation.

Along with our presenters, Jane will help participants contemplate what matters as we near the end of our lives, and explore local services that can enhance that experience for the patients and families we care for.

Prof Liz Reymond

PhD, FRACGP, FAChPM

Liz has been a palliative care physician for 20 years. Over ten years she established Queensland’s largest multidisciplinary specialist palliative care service in Metro South Health. Attached to that service, she developed the Brisbane South Palliative Care Collaborative, the research and service development arm of the clinical service. Currently, the Collaborative is running two national palliative care grants: caring@home and Advance Care Planning Australia. She is currently Director of Queensland Voluntary Assisted Dying Support and Pharmacy Service.

Liz will be providing a keynote presentation on home based palliative care at the General Practitioner dinner as well as speaking to national initiatives and participating in panel discussions at the Education Day.
