Join one of the largest quality improvement programs in Australian primary care

The Heart Foundation program aims to support general practice with the targeted identification and management of CVD via an efficient SMS recall tool that is embedded within CAT4.

Two hundred practices are being offered the opportunity to participate in the Text to Detect program. Participating practices will have access to one-to-one support from the Heart Foundation and will be supported by practical tools and resources that will help them conduct and streamline Heart Health Checks.
