The PHN Blue Sky Fund is designed to provide flexibility in capturing opportunities to trial projects that can deliver tangible positive health outcomes for the community.
The Small Town Engagement Model (STEM) aims to support small rural towns with less than 5000 people. This model is where an existing allied health business hosts another primary care provider that creates a Multidisciplinary Team and provides a boarder scope of services to the small community.
The Health Hub will be developed alongside the CARE Model in Glen Innes to ensure the local community and health professionals lead the implementation of the CARE model in their community.
Please join us for an information session on the Social Futures service Veterans Connect. Veterans Connect is a service that connects former Australian Defence Force (ADF) members and their families to the right health, wellbeing and community supports on the Central Coast. Veterans Connect can link veterans, their families and carers to a wide range of supports to assist with the timely receipt of appropriate healthcare.
HNECC PHN has worked with Uniting, an NDIS partner in the community on the development of this information session. We will discuss NDIS updates, referral pathways, and how to navigate these changes.
