The Health Hub will be developed alongside the CARE Model in Glen Innes to ensure the local community and health professionals lead the implementation of the CARE model in their community.
The PHN will also commission an operator to deliver the Glen Innes CARE model services to the community. Through collaboration with the Glen Innes community, health professionals and local service providers, a governance structure will be developed to clearly define the Health Hubs role, as well as the role of local service providers that will be commissioned by the PHN.
The IMOC pilot funding is a four-year pilot program, that is funded to June 2027.
