Aged Care Information and Services
The PHN provides links and resources to groups that advocate for older people and general information about the health and wellbeing of, and care for, older people.
These resources include the MyAgedCare website, Australian Government Department of Health and Aged Care website, PatientInfo and the Services Australia Aged Care Provider Portal.
All Aged Care services can now be accessed through the MyAgedCare website, which includes a service finder feature where an individual or their carer can look for local meal, support and care coordination services, and much more.
