Glen Innes Health Hub Pilot

The Hunter New England and Central Coast Primary Health Network (the PHN) has successfully secured pilot funding under the Federal Government’s Innovative Models of Care (IMOC) Program, which will see Glen Innes as a pilot site to introduce an innovative healthcare model.

The pilot focuses on collaborating with local communities and healthcare professionals to tailor a multidisciplinary solution that aligns with and benefits the local community, potentially introducing new services or enhancing existing ones. The Health Hub services will potentially encompass a care navigator, allied health, social services, aged care, disability, and primary care services, including GP care.

The Health Hub will leverage innovative, digitally enabled solutions.

Through collaboration with the Glen Innes community, health professionals and local service providers, a governance structure will be developed to clearly define the Health Hub’s role, as well as the role of local service providers that will be commissioned by the PHN.

The PHN is seeking Providers interested in being a part of this Innovative Model of Care pilot in Glen Innes, NSW.

The exercise will be a two staged process, initially with an expression of interest (EOI) submission to pre-qualify respondents to participate in Stage 2 of the exercise. Stage 2 of the exercise will be open only to those pre-qualified organisations from the Stage 1 EOI and will be a formal Request for Proposal (RFP) seeking organisations with capacity and experience to deliver services as part of the Model of Care pilot in Glen Innes.

The EOI and Stage 2 Request for Proposal (RFP) will be released via Tenderlink.

A Provider information session for organisations interested in participating in the expression of interest will be held on Thursday 14th March online via Microsoft Teams.
