Awards criteria

Noting for each nomination there is a requirement for:

Submission close 15 April 2024 at 5pm.

Patients First Award

Awarded to healthcare organisations, clinics/teams and/or service providers that put people and communities first in their practice: they demonstrate engagement strategies to support best practice outcomes for the health care organisation, the community, and their patients.

A patient is defined as an individual who accesses primary care and could be considered a client or consumer as well.

Please tell us in more detail how you or your organisation put patients first within your work. Please address the following in your nomination:

  • What problem were you addressing, and how did you prioritise patients?
  • How did the patient input shape your approach and what impact was observed?
  • Describe partnerships and the role of Health Pathways (if used) in care delivery?
  • What outcomes were achieved, and can you provide data demonstrating impact?
  • How has this delivered on increasing equity of access to primary care services?
  • What are your plans for sustaining and developing this patient focus?

First Nations Health Award

Awarded to the healthcare organisation, clinic/team and/or service provider that demonstrates being responsive to the diversity of, and differences in, our communities to address health inequalities for First Nations people and communities (including but not limited to increased incidence of identification and 715 health checks).

Please tell us in more detail how your work has assisted First Nations people and communities achieve better health outcomes. Please address the following in your nomination:

  • What problem were you trying to solve, and what was your solution or initiative to promote better health outcomes for First Nations people?
  • What was the impact, and can you provide data to demonstrate the outcomes?
  • How have you consulted with First Nations communities in the design of this work?
  • How does your organisation support and encourage your First Nations and Torres Strait Islander workforce or new recruits?
  • How has this delivered on increasing equity of access to primary care services?
  • [For non-First Nations organisations] What cultural awareness/training or responsiveness do you embed in your organisation?

Technology in Health Award

Awarded to the healthcare organisation, clinic/team and/or service provider that demonstrates leadership in the use of technology to support their patients. A patient is defined as an individual who accesses primary care and could be considered a client or consumer as well.

Please tell us in more detail how you or your organisation has utilised technology to support your patients. Please address the following in your nomination:

  • What problem or issue did you address, and how did you leverage technology for a solution?
  • In what ways did you use technology to assist and improve patient outcomes?
  • What measurable impact did the technology have on patients, and what were the outcomes?
  • Share data/evidence demonstrating improved clinical outcomes and/or more efficient work practices due to the implemented technology.
  • Will this technology be embedded in your practice, and do you have plans to further develop and grow?
  • How has this technology delivered on increasing equity of access to primary care services?
  • How have you ensured accessibility and inclusivity of the technology to diverse patient populations and varied digital literacy?

Primary Care Innovation Award

Awarded to the health centre/clinic/team and/or service provider that demonstrates outstanding innovation in primary care.

Please tell us about new practices or projects that you have introduced to your work that were innovative and assisted with improving health outcomes. Please address the following in your nomination:

  • Describe the specific innovative practice, project, or approach you introduced to improve health outcomes.
  • What problem were you trying to solve with this innovation, and why was it significant for your community or patients?
  • Explain your solution and highlight what sets this approach apart from traditional methods.
  • What impact did the innovation have on patients, and can you provide data demonstrating the outcomes?
  • How has this solution delivered on increasing equity of access to primary care services?
  • How has this innovation been integrated into your work or organisation, ensuring sustainability and continuous improvement?

Primary Care Leader Award

Service Provider and Clinic/Team and Individual

Awarded to the health centre/clinic/team, service provider or individual that demonstrates outstanding innovation in primary care.

Please address the following in your nomination:

1. Explain how you, whether individually or as an organisation, or clinic/team have contributed to:

  • Developing flagship services for communities; or,
  • Developing prevention and self-management strategies; or,
  • Developing and implementing new models of care; or,
  • Embedding continuous quality improvement processes and strategy within the business.
  • Delivering on increasing equity of access to primary care services.

2. Describe a leadership initiative that directly contributed to improving health outcomes in your primary care role or organisation.

3. Explain your role in fostering a positive workplace culture that enhances the effectiveness and well-being of your primary care service or team.

4. Share an example of how your leadership has driven innovation in primary care services, leading to better patient experiences or outcomes.

5. Provide evidence or examples showcasing the impact of your leadership on enhancing overall organisational performance and patient care.

  • Clarity in describing the situation and demonstration of effective leadership
  • Demonstrated leadership impact and connection to health outcome improvement
  • Demonstrated impact on team effectiveness and well-being
  • Leadership-driven innovation and Impact on patient experiences or outcomes

Primary Care Dedication to Service Award

A peer nominated award for an individual who has displayed outstanding dedication to providing quality primary care over many years. The individual is well respected by peers and patients and actively participates in the community and professional bodies. These individuals deserve an honorary place in our region’s Primary Care Hall of Fame.

Please describe why this individual deserves to be nominated for the Primary Care Dedication to Service Award.

  • Description of the nominee's significant contributions to the primary care sector in our region over the past 10+ years
  • Highlight specific achievements or milestones that showcase the nominee's exceptional impact on the primary care sector
  • Provide details of any awards the nominee has received for their dedication to service in primary care
  • Share any de-identified case studies that exemplify the nominee's work and commitment in the primary care sector
