The prevalence of diabetes in Australia is rising, affecting around 1.2 million people in 2014-15. In New South Wales in 2016, 8.9% of people aged 16+ were told they had diabetes or high blood glucose levels, up from 6.5% in 2002.
Hunter New England Central Coast PHN is pleased to offer practices in the region the opportunity to take part in this new initiative focusing on patients with chronic disease who are at high risk of complications and/ or hospitalisation.
