After Hours Needs Assessment and Commissioning Project

To improve access to After-Hours Primary Health Care across our region we recently completed a comprehensive Needs Assessment process.
As a result of findings within the Needs Assessment we have recently conducted an Expression of Interest (EOI) process to identify organisations who have the capacity, capability and experience to deliver After Hours services to communities across our region.
Our aim is to explore more fully how current and new models of after-hours care can connect with patients’ existing GPs to promote continuity of care. This would include where possible establishing protocols to transfer care back to a patients’ regular GP.
This process will not impact on existing after hours services funded by the PHN and current provider contracts will continue until June 30, 2021.
