End of Life Care

The Greater Choice for at Home Palliative Care measure (GCfAHPC) provides funding for coordinating palliative care through PHNs. As being cared for and dying at home is most people’s preference, the initiative aims to help make that possible. A recent Palliative Care Needs Assessment identified the need for an End of Life Coordinator, a role that will be commissioned in a pilot program in the Hunter region.

To help health care professionals develop resilience, feel good and function effectively, the Hunter New England and Central Coast Primary Health Network (HNECC PHN) is inviting primary care professionals and their families to participate in a unique wellbeing initiative called The Lift Project.

Safe and Healthy: Improving Primary Care’s Response to sexual abuse and child sexual assault

As part of the Commonwealth funded Family, Domestic and Sexual violence pilot initiative, the PHN will procure additional services to deliver an integrated model of support for victim-survivors of sexual violence and child sexual abuse (SV and CSA). These services will closely link to existing family and domestic violence services commissioned by the PHN and increase the capacity and capability of primary care to better care for victim-survivors. The scope of these services will be co-design ahead of the Request for Proposal (RFP) being released in November 2023.

HNECC PHN Selected to pilot the new model of trauma-informed care, supporting victim-survivors of family, domestic and sexual violence.

The Hunter New England and Central Coast Primary Health Network (HNECC PHN) is one of six PHNs that have been selected to pilot a new model of trauma-informed care to support victim-survivors of family, domestic and sexual violence commencing in 2024.
