With a focus on engaging and empowering patients, activating caregiver networks and understanding that patients are increasingly behaving as consumers of healthcare. Digital health provides us with a toolbox of technologies and techniques that support the development of new, innovative patient and caregiver-centred models of care.
Health-e Together Digital Care Toolbox.
This change management toolbox offers healthcare providers a range of resources, tools and techniques to support the implementation of digital change at different levels of digital health readiness.
The toolbox will assist healthcare organisations to recognise how digital health technologies are applied in practice to achieve greater efficiency in healthcare delivery and better health outcomes for consumers.
Digital Care Survey.
The brief survey is an effective tool for healthcare services to identify gaps in their practice digital health readiness and be part of the change approach for future digital health initiatives. Participating in this survey helps healthcare practices determine their digital health readiness level and supports the planning for innovative digital health activities in the region.
