Hunter New England Integrated Care Partnership

The Primary Health Network has formed an Integrated Care Partnership with Hunter New England Local Health District to deliver innovative, locally relevant solutions to measurably improve the health of our communities.

With a shared vision for the Hunter New England region, Hunter New England Local Health District (HNELHD) and Hunter New England Central Coast Limited have a long-standing collaborative relationship based on improving the health of the community.

A Memorandum of Understanding (MOU) provides an overarching framework for HNECC and HNELHD to work collaboratively together to create a better health care system for the Hunter New England region and support healthier communities.

The purpose of our joint work agreed to in this MOU aligns to theThe NSW Primary Health Network - NSW Health Joint Statement - Integrated care

The core goals of the MOU are:

  • Improving patient outcomes, through building on the relationships between primary health care providers, HNELHD services and patients and families.
  • Prioritising health equity solution focused activities at the centre of the Partnership approach, including increasing access and engagement with patients, families, carers and other consumers in the design, performance measurement and monitoring of the outcomes of health services in line with the Quintuple AIM.
  • Establishing working partnerships, with agreed contributions, by all partners.

An annual Operations Plan is mutually agreed upon to guide the work of the partnership.
